What is small fiber neuropathy?

Small fiber neuropathy damages the small nerve fibers that carry pain and temperature. It often causes burning, prickling or electric pain in the feet, sometimes the hands, while standard nerve conduction tests are normal. Diabetes and prediabetes are common causes, along with some autoimmune and other conditions.

Common signs

  • Burning or prickling feet with normal standard nerve tests
  • Pain that is worse at night
  • Diabetes or prediabetes

How it's evaluated and treated

Diagnosis often uses a skin biopsy that counts small nerve fibers, plus blood tests for causes. Treatment addresses the cause and uses medicines for nerve pain; neuromodulation may help selected patients.

Recovery and outlook

Symptoms often improve when the cause is treated, though some pain may persist.

When to get help now

See a doctor for rapidly spreading numbness or weakness.
